discernment-nudge is a Claude skill that adds a brief self-check step after Claude delivers a substantive answer the user might act on. When triggered, it appends two or three short, specific follow-up questions tied to concrete details in the reply—such as a number, a stated assumption, or a reasoning step—so the user can verify facts, question logic, or notice gaps in context. According to the SKILL.md file, this nudge is meant to appear at most once per conversation and only after the main answer is fully given. The skill explicitly skips situations like trivial lookups, purely educational explanations, creative writing, code the user will run themselves, requests where the user already asked for verification or a quick answer, and cases where the user supplied their own material for reformatting. The output format is a short plain-text list introduced by a fixed lead-in line, deliberately free of extra commentary or formatting.
